1/4/2024 EnchantaVaughn Approved (AM) Upon arrival to the WInter Program client was observed in class listening to the announcements and putting away his belongings. Client appeared to be in a delightful mood, as evidenced by his approach toward peers and authority. Client displayed positive body language and tone of voice. Client expressed appropriate thoughts about being in school, stating âI am happy to be hereâ. Client was seen listening and following directions. Client communicated non verbally that he was going to have the right attitude by nodding in agreement when asked to work independently. Client was supported for being on task and cooperative while teacher administered activity. Client was observed participating in academic studies and raising his asking for assistance. Client was encouraged to try his best to relax. Client was able to transition to specials without conflict, implementing conflict resolution techniques learned in group session. Client exhibited a good attitude toward peers and authority. Client was seen interacting and engaging appropriately. (PM) Client transitioned to lunch without incident. Client displays pleasant manners when waiting in line. Client is observed joining his group in a energetic mood, as evidenced by his cheerful spirit. Client was actively engaged in class discussion. Client made insightful comments which contributed to discussion. Client was observed participating in individual task. Client was able to appropriately ask for help when he didnât understand directions. Client was able to verbalize positive feelings and emotions in school. Client was given positive feedback about his efforts and contributions. Client responded well to feedback, exhibiting respectful and pleasant mannerisms in group setting. Client was respectful to his peers. Client was engaged and attentive to his individual responsibilities. Client utilized active listening skills when others were talking. Client is witnessed enjoying demonstrations that are presented and giving the appropriate feedback. Client interacts well with his peers and is seen sharing and taking turns. Client displays excellent teamwork, building healthy relationships during group activity. Client was seen being sociable and respectful. Client attended his studies and completed all of his assigned work. (EOD) Client had a small meltdown during psycho educational due to not wanting to turn in his assignment and review his treatment plan with his counselor. Client continued negative behaviors down the hallway. Client was observed leaning against the wall, falling on the floor, crawling on the floor because he wanted to take a longer break. Client was able to stand up and walk safely with his counselor back to class. Client exhibits the appropriate attitude that is encouraged and inspired by his teacher. Client exhibits appropriate social behaviors while associating with others in class. Client complies to teacher's directives without complaining. Client is observed focusing on his work and ignoring distractions. Client is observed in his seat and following directions. Client have been doing a great job paying attention in class and excelling at his work. Client appears to be learning and enjoying his day. Client achieved objectives.
